Of the range killers, The Plague is probably the easiest to grasp and begin to master. Her ability is infection through vomit, which she projects into the air to spread disease to objects and survivors. Survivors hit by the vomit will become sick. They can also become ill when they interact with infected objects, such as pallets, generators, totems, windows, and even other survivors.
To recover from the sickness, survivors can cleanse at the Pool of Devotion (more on this below). When a survivor does not cleanse, they will eventually lose a health state and become broken, which means they cannot be healed. Because of this, you should try to keep everybody sick.
Survivors who are infected will display a yellow/green stain over their image in the HUD (Heads Up Display). Those who are broken will have a red stain and a plus sign in a circle with a line through to the right of their character icon (see image below).

The Plague has Two Types of Vomit
Survivors can cleanse at one of the Pools of Devotion scattered throughout the map. When they cleanse, their sickness is transferred to the water at the Pool of Devotion, causing it to turn red. When The Pool of Devotion is red, survivors cannot use it to cleanse. The Plague, however, can drink from it. When she does this, her Vile Purge becomes Corrupt Purge for 60 seconds and can down a survivor. Keep in mind that when survivors cleanse at all the Pools of Devotion on the map, it resets them. The Plague will automatically gain Corrupt Purge, but once it times out, you’ll have to wait for a survivor to cleanse to regain it.
Much like The Plague’s basic attack, hitting them with her censor, the Corrupt Purge must land on a survivor twice to down them. Below is a video that shows a survivor being downed by Corrupt Purge.
Arcing Vomit
To reach over walls and increase your reach, try to arc the vomit so that it goes over walls to land on generators or survivors. You can also move your camera left and right to hit more land mass.
Below is a video to show how far vomit can reach when projected into the air. I’m still not very good on getting this right, it takes time and practice.
Tips for Infecting Objects
Before starting the game, you need to come up with a strategy for spreading your disease. Basically, what will you puke on first, objects or survivors. If you choose to infect objects, a survivor must interact with it in the first 40 seconds, after that it is no longer infected. Because of this, you may want to use one or two of the following Add-Ons to increase the duration.
- Limestone Seal: increases the duration of infected objects by 20 seconds.
- Prayer Tablet Fragment: increases the duration of infected objects by 20 seconds. The survivor will also spread the disease 100% faster when interacting with other objects or survivors. The downside is that you will no longer be able to infect survivors by puking on them.
- Haematite Seal: increases the duration of infected objects by 30 seconds.
- Infected Emetic: increases the infection gained by 40%
Tips for Infecting Survivors
If you want to infect survivors, The Plague has two good information Add-Ons to help you find them.
- Olibanum Incense: when a survivor cleanses at the Pool of Devotion, their aura becomes visible to you for 4 seconds.
- Black Incense: when a survivor vomits, their aura is revealed to you for 3 seconds.
Add one or more of the information perks listed below and you’ll have a good time finding survivors. Lethal Pursuer is a favorite of mine since it not only shows you where the survivors are at the start of the game, but it increases every visible aura by 2 seconds.
- Alien Instinct (Xenomorph): when you hook a survivor you’ll see the aura of the furthest infected survivor for 8 seconds.
- Darkness Revealed (Dredge): when you open a locker, you’ll see the aura of survivors within 8 meters for 6, 7 or 8 seconds, depending on the perk tier.
- Infectious Fright (The Plague): When a survivor within your terror radius goes into the dying state, they will scream and reveal their location for 4, 5, or 6 seconds depending on the tier level.
- Iron Maiden (Legion): When a survivor exits a locker, they will scream and reveal their location.
- Lethal Pursuer (Nemesis): You’ll see the auras of all survivors for 7, 8, or 9 seconds depending on the tier. This will also extend the effects of survivor auras that are revealed by 2 seconds.
Other Add-Ons and Perks
There are other Add-Ons and Perks to help you achieve your goals and win, so take some time to peruse what’s available to you. Also, since the number one goal for survivors is to repair five generators, you may want to bring along a generator regression Perk or two. The Plague comes equipped with Corrupt Intervention, which blocks the three furthest generators for 80, 100, 120 seconds depending on the perk tier.
